Essential forms for players and staff

Here you will find a comprehensive collection of forms necessary for participation and reporting within the South Alabama Youth Football League. These documents are crucial for ensuring the safety and proper conduct of all our activities, from player registration to incident reporting. Please ensure all forms are completed accurately and submitted as required.

Player injury and incident reports

In the interest of promoting player safety and maintaining accountability, the following forms may be used to report player injuries and Code of Conduct violations that occur during a SAYFL event or game.

The Player Injury Report Form may be submitted by a player’s parent or guardian, coach, or team president.

The Code of Conduct Incident Report Form may be submitted by any individual who witnesses a violation.

We encourage all incidents to be reported promptly and accurately to help SAYFL maintain a safe, respectful, and positive environment for all players, coaches, families, officials, and spectators.

Cheerleading competition waivers

All cheerleaders participating in the cheer competition who will be competing outside of their designated age group are required to have a properly completed Cheer Competition Waiver on file.

The completed waiver must be submitted by the established deadline to the participant’s Organization President or designated Committee Representative.

By completing and submitting this waiver, the participant and their parent or legal guardian acknowledge and accept the rules, requirements, and safety guidelines associated with competing outside of the participant’s designated age group. This requirement is intended to promote participant safety, minimize potential risks, and help ensure a fair, organized, and successful competition experience for everyone involved.
